Position Summary
The Re-entry Director, reporting to the AVP, Residential, leads the development, implementation, and growth of community reintegration and alumni support programming within our residential programs. This role bridges residential care and sustainable independent living by designing structured re-entry pathways, leading group sessions, and fostering an active alumni network in close collaboration with Residential Program Directors.
Key Responsibilities
- Facilitate evidence-based group therapy and educational sessions focused on reentry, relapse prevention, life skills, job readiness, and community integration.
- Design and refine structured reintegration workshops and transition plans tailored to individuals transitioning out of residential care.
- Provide high-level guidance and complex case oversight for residents preparing for discharge and long-term community recovery.
- Build, launch, and manage an active Alumni Network to maintain long-term engagement, peer support, and mentorship opportunities for program graduates.
- Organize regular alumni gatherings, recovery support meetings, and community reintegration events.
- Maintain engagement tracking and longitudinal data to evaluate post-discharge success and inform program enhancements.
- Partner closely with Residential Program Directors to align discharge planning with incoming reentry initiatives, ensuring warm handoffs and continuity of care.
- Coordinate with clinical, case management, and vocational staff across residential sites to streamline reentry pathways.
- Ensure all group activities, documentation, and operational protocols align with OASAS standards and organizational guidelines.
- Work closely with clients and their counselors to assess housing needs, coordinate housing resources and referrals, and support successful placement into safe and stable housing.
- Conduct community-based outreach and support with clients for up to 30 days following discharge, helping address barriers, connect to resources, and promote continued success in meeting reentry goals.
Education:
- Bachelor’s Degree in Human Services, Psychology, Social Work, or a related field required. Candidates with extensive (7+ years) relevant professional experience may be considered.
- Master’s Degree (MSW or MHC/LMHC) strongly preferred (LMSW, LCSW, or LMHC active NYS license is a significant plus).
Experience & Skills:
- 5+ years of progressive experience in addiction treatment, behavioral health, or reentry services.
- Demonstrated track record of group facilitation and curriculum implementation.
- Experience working within OASAS-certified or licensed residential settings.
- Leadership, networking, and community-building capabilities.
- In-depth knowledge of trauma-informed care, person-centered planning, and harm reduction principles.
- Excellent interpersonal skills to foster partnerships across residential leadership teams.
